Insurance estimates and your share of the cost
Before treatment, our team will review your estimated insurance benefits and expected out-of-pocket costs with you. An estimate is based on the information available from your insurance company; it is not a guarantee of coverage or payment.
Your plan's deductible, annual maximum, exclusions, and final claim decision can affect what your insurer pays. You are responsible for amounts your plan does not cover. We file claims for you and can help explain the insurer's response and any remaining balance.
